Post-Mortem Markdown Structure
The generated post-mortem is crafted specifically for developer review in GitHub Issues and Pull Requests:
- Metadata Header: Trace ID, supervisor PID, timestamp, exit code.
- Executive Summary: High-level failure verdict.
- Deterministic Findings Table: Exact rule codes, severities, affected tools, and line numbers.
- Chronological Evidence Trail: Clean timestamped log of tool events and errors.
- Local AI Hypothesis (if enabled): Contextual root cause explanation from Ollama.
- Remediation Checklist: Actionable tasks for human engineers to prevent regressions.
